Step 1: Access the Official Grievance Portal

First, open a web browser on your phone or laptop and go to the official PM-JAY portal at pmjay.gov.in. On the homepage, look for an option labeled “Grievance Portal” and click on it. This will direct you to the page where you can submit your request.

Step 2: Register a New Grievance

On the Grievance Portal, you will see several options, including “Check Your Eligibility,” “Register Your Grievance,” and “Track Your Grievance.” To begin the process of adding your name to the list, click on “Register Your Grievance.” This will open the grievance registration form for the PM-JAY scheme.

Step 3: Fill Out the Grievance Form Details

A form will appear that you must fill out carefully. Follow these instructions for each section:

  • Grievance By: Select “Beneficiary” from the options.
  • Case Type: Choose “Request.”
  • Are you enrolled under AB-PMJAY?: Select “No.”
  • Personal Details: Enter your full Name, Gender (Male/Female), Year of Birth, and a valid Contact Number.
  • Location: Select your State and District from the dropdown menus.
  • Address: Provide your complete address, including your village and post office. You can also add your email address if you have one.
  • Grievance Details: In the ‘Grievance Against’ section, select the appropriate authority, such as the District Grievance Nodal Officer. For ‘Nature of Grievance,’ you must choose “Eligibility.”

Step 4: Write Your Grievance Description

The ‘Grievance Description’ field is the most critical part of the form. Here, you need to explain your situation clearly in either English or Hindi. You should state that you belong to a very poor family and are eligible for an Ayushman Card, but your name has not been included in the list. Politely request that your card be issued.

Step 5: Upload Supporting Documents

You are required to upload supporting documents to validate your claim.

  • Supporting Document: Upload a clear copy of your Aadhaar Card, as it is the primary document used to issue an Ayushman Card.
  • Audio/Video (Optional): There is an option to upload a short audio or video file (40-50 seconds). In this file, you can verbally state the same request you wrote in the description box, mentioning your name and Aadhaar number. This step is optional and not mandatory.

Step 6: Submit and Track Your Grievance

After filling in all the details and uploading your Aadhaar Card, click the checkbox to give your consent and then press the “Submit” button.

Once submitted, your grievance will be sent to the government, and you will receive a Grievance Number. Keep this number safe. You can use it to track the status of your application by returning to the Grievance Portal and clicking on “Track Your Grievance.” It typically takes about 7 days for your name to be added to the list after your request is processed.

What Happens After Approval?

If your grievance is found to be legitimate, the status will update to “Success.” This confirms that your name has been added to the Ayushman Card list. Once your name is on the list, you will be able to generate your own Ayushman Card using your Aadhaar Card and avail the benefits of the scheme.
